To check a community’s licensing status, visit the human services provider directory maintained by the Pennsylvania Department of Human Services. Under program office, select Office of Long-Term Living. Then, choose from personal care homes and assisted living facilities in the service code drop-down menu. Finally, select secure dementia care unit under the service code sub category menu. In the license status and license number column, you’ll see links to an inspection summary for each community. Clicking on inspection summary will take you to all available inspections. You can see if a community passed their last inspection, or if they didn’t pass, you’ll see details on the specific violations.
